CareAligned is a practical tool for seeing the caseload, planning visits, and staying organized.
What is available today?
The printable Weekly Care Planner is available now. It includes space for planned visit days, supplies, medication refills, recertification timing, notes and needs, calls and tasks, and reminders. Additional tools are being prepared and are not yet available.
What are the upcoming tools?
CareAligned is developing blank continuity-style planning sheets and practical calls, tasks, and reminder tools. We will share them when they are ready.
Does the preview use real patient information?
No. The planner is intentionally blank and privacy-safe. It uses neutral labels only and does not include patient identifiers or clinical examples.
Is CareAligned an EHR or compliance system?
No. CareAligned is a personal planning and organization aid to use alongside the employer-approved systems and processes required by your organization.
Will the weekly planner print clearly?
Yes. The worksheet is formatted for US Letter landscape paper with clean, black-and-white legible grid lines when printed.
