Why I started making these forms.

A note from Rachel

Welcome.

When I transitioned from hospital nursing to home health, I realized quickly that staying organized wasn’t just helpful, it was essential. I needed a way to see everything at a glance, so I started creating my own forms. Simple, practical tools to keep me organized and focused.

I’m sharing them because I know how overwhelming home health can feel, whether you’re new or you’ve been doing it for years. My goal is to make your days a little easier and to help you focus on what matters most: patient care.

Because organization doesn’t replace great nursing. It supports it.

Created by a home health nurse, for the nurses who give their all every day.

What the pages are for

Practical care planning, organized.

The Weekly Care Planner keeps the caseload, planned visit days, supply dates, refill needs, recertification timing, notes and needs, important calls and tasks, and reminders in one working view.

CareAligned is a planning and organization aid—not an EHR, charting, documentation, clinical decision, or compliance system. Keep identifying information out of the blank pages.
